Confirmation of Qualifications - Duplicate Diploma. The fee includes post and packaging for the UK only.

Please specify the full date ie. 21/10/1958 and the name you qualified in, in the notes field on the next page otherwise your request will be delayed.

NB that your diploma will not be sent until we have received your sworn affidavit.
