Confirmation of Qualifications - Duplicate Diploma. This fee includes post and packaging for those based outside of the UK and Europe.

Please specify the full date ie. 21/10/1958 and the name you qualified in, in the notes field on the next page otherwise your request will be delayed.

NB that your diploma will not be sent until we have received your sworn affidavit.
